Well it turns out the monitor is a 15AV as seen here. As for the video card, it's only labelled as "High Resolution Display Video Card". It's in rather bad shape. There's 2 capacitors missing from the board, of unknown values. It's not one of the portrait cards; there's one of those in the IIci that I have, it has the strange connector on it, this one doesn't.
