I format my CFs through a PowerBook (190, 5300ce or 1400c) using a CF to PCMCIA adapter and it formats fine with System 7.5.5, 7.6.1 and 8.1 (respective to the machines). Drive Set Up used on these Systems work just fine in partitioning and formatting the CFs. It also works as great on my WallStreet G3s.
I also use a Multi-Media Card Reader (SD, CF, etc.) to USB adapter on my 12inch G3/G4 iBooks & PowerBooks on OSX 1.4.11 running OS 9.2. I also have a 17inch G4 PowerBook with a single PCMCIA Slot and it reads and writes CFs with ease with the PCMCIA Adapter, though I have not formatted or installed CFs on that one yet.
I'm hoping that you do not have a hardware issue with the CC. Do you have a PowerBook 68K/PPC/G3/G4 with a PCMCIA or USB options?
What settings is the Aztec Monster on and what CF brand are you using?
