You should be greeted immediately with "APPLE SSC:" when you do Ctrl-A.  The fact that it doesn't means the card is bad, the slot is bad, or maybe it's configured for printer rather than modem use.  Have you checked the DIP switches?

Yes.  This will only help you if your SSC actually works, though.
