Success! I forgot to do one step
I did not sell my soul to the Devil. Now that I did that, it works!!
Ok. Not really sure what the issue was after the last round of low formatting. But I did notice that there were, for whatever reason, several small partitions. I did not pay attention since they were small driver partitions. My guess is that was the issue.
This last time, I low level formatted the drive (again) on the 5215 and partitioned it into two parts. This means I lost about 600MB of the capacity since its a 4.5GB and 2 is the max. But this time I also noticed just three areas of the drive, one for driver and two for partitions. Jumpers are all default, SCSCI ID is 0. I used the cable that was in the SE. This cable has a cut wire (small section missing) in the ribbon. Not sure what its for, but I am using it and it works.
Now to install a system and get this thing producing floppies!
