Most of the iTunes interface is hard coded so its really hard to theme it. Pretty much everything bar the brushed metal can be changed dramatically

It seems Skin a dock no longer works...what do you use?

An alpha of Unsanity's DockMod. Actually, you can try using A-Dock, which is a total dock replacement, but supports skins even better.
