According to this link http://docs.info.apple.com/article.html?artnum=112195, the IIvi can only do 512 x 384 or 640 x 480. Your monitor also needs to support a refresh rate of 67 Hz - which was the old Mac standard.

With rasterops adapter I cannot get 640 480 resolution in my VGA monitor eventhough monitor supports 67 Hz.
